Alcoholism is endemic in Nigeria’s traditional religion and society. This abuse is especially common at New Yam festivals, Ekpe, Ekpo and Nmanwu masquerades festivals, burial rituals, birth, marriage and naming ceremonies. Some claim that this is driven by specific beliefs and activities in African culture, such as beliefs in ancestors, libation, hospitality and entertaining guests and strangers and the desire to maintain the cultural traditions of the ancestors. Alcohol abuse has generated major health and social issues for abusers, their families (...) and society, plunging families, towns and tribes into crises and conflicts that bring economic and political retrogression. This research studied how the African traditional religion encourages alcohol misuse and how to decrease it for national development. This study was on Nigeria’s South-South region. The study uses qualitative and ethnographic research methodologies, including key informants, in-depth and focus group interviews and the reward deficiency syndrome as a theoretical framework. Although African Traditional Religion (ATR) supports alcohol usage, greed, a lack of self-control, peer pressure, indiscipline and lack of moral upbringing led to alcohol misuse, which harms the person, family, community and country as a whole. Education and enlightenment are a remedy to free alcoholics and utilise them for national integration and development.Contribution: Some say Africans drink a lot because their religious heritage promotes drinking, leading to abuse. However, peer pressure, selfishness, a lack of self-control, bad parenting and not religion push persons with reward deficiency syndrome into alcoholism, according to this research. (shrink)

In this series of eight articles, the Australian Traumatic Brain Injury Initiative (AUS-TBI) consortium describes the Australian approach used to select the common data elements collected acutely that have been shown to predict outcome following moderate-severe traumatic brain injury (TBI) across the lifespan. This article presents the unified single data dictionary, together with additional measures chosen to facilitate comparative effectiveness research and data linkage. Consultations with the AUS-TBI Lived Experience Expert Group provided insights on the merits and considerations regarding data (...) elements for some of the study areas, as well as more general principles to guide the collection of data and the selection of meaningful measures. These are presented as a series of guiding principles and themes. The AUS-TBI Aboriginal and Torres Strait Islander Advisory Group identified a number of key points and considerations for the project approach specific to Aboriginal and Torres Strait Islander peoples, including key issues of data sovereignty and community involvement. These are outlined in the form of principles to guide selection of appropriate methodologies, data management, and governance. Implementation of the AUS-TBI approach aims to maximize ongoing data collection and linkage, to facilitate personalization of care and improved outcomes for people who experience moderate-severe TBI. Harris, and Jonathan A. SeitzThe Tenth Conference of the European Network of Buddhist-Christian Studies (ENBCS) brought together between sixty and seventy people at the Oude Abdij, Drongen, Belgium, between 27 June and 1 July 2013, to examine the theme “History as a Challenge to Buddhism and Christianity.” It (...) was a collaborative effort between the ENBCS and the Katholieke Universiteit Leuven. The 2011 ENBCS conference had focused on whether hope was a relevant concept for Buddhists and Christians. The choice of history as a theme complemented this. It also resonated with a research concern of both KU Leuven and the Network.Five key themes were chosen for examination: the traditional Christian and Buddhist conceptions of history; Jesus and the Buddha: fact and fiction; Buddhist and Christian historiography; dangerous memory in Christianity and Buddhism; and permitting historical consciousness in Buddhism and Christianity. Each was approached from a Buddhist and a Christian perspective. An opening lecture addressed the theme “Modern Historical Consciousness and the Challenge to Religion.” In addition, eighteen research papers on Buddhist-Christian relations were given in parallel sessions on Friday evening and Saturday morning. These covered topics as various as the writings of Edmund Pezet (Pierre Gillet), Paul Knitter’s understanding of Buddhist-Christian double belonging (Achim Riggert), Keiji Nishitani and the death of God (Jonatan Navarro Estrada), the encounter of Zen Buddhism and Christianity in the works of D. T. Suzuki (Mariusz Rucki), Buddhist-Christian encounters by Han Yong Woon (Seung Chul Kim), and “Liberating the Conflict from ‘History’ in Sri Lanka” (Jude Lal Fernando).Perry Schmidt-Leukel (Münster, Germany), in the opening lecture, discussed [End Page 189] seven challenges that modern historical consciousness was posing to religion: the challenge of falsification (when historical evidence, for example, showed some doctrines cherished by believers were not taught by the founder), the challenge of uncertainty or methodological doubt, the challenge of methodological naturalism (should a natural explanation be considered more probable than a supernatural one?), the challenge of the distance between past and present, the challenge of change (religions are not static), the challenge of demystification, and the challenge of relativity (the writing of history is perspectival, conditioned by context). He stressed that it would be a blessing to religion if these challenges were faced and, throughout the conference, encouraged other speakers to face them.The first theme was tackled by Mark Blum (Berkeley, USA) from a Buddhist perspective and Jan-Olav Henriksen (Norwegian School of Theology) from a Christian perspective. In a magisterial historical and geographical survey of the Buddhist tradition, Blum first noted that no one had unearthed a traditional notion of Buddhist history in the early texts. The theme of decline and reformation had been taken from Indian cosmology, but with a Buddhist twist and the conviction that it was pointless to talk about beginnings. And in the growth of the Mahāyāna tradition, a dehistoricization of the Buddha had occurred in the belief that the Buddha’s dharma-body was his only “real” body.Henriksen argued that Christian salvation history had its origin in the narrative of the Fall, an event for which there was no historical evidence, and developed into a history of God’s call (election) to all people that they should be brought together in community. It was a history that was linked with the divine logos, a force that Christian theology has seen as deeply interwoven into creation from the beginning of time and that was made human in Jesus.Terrence Merrigan (KU Leuven, Belgium), giving a Christian perspective on the second theme, distinguished three quests or attempts to reach the historical Jesus. The first, from 1774 to 1906, sought to uncover the Jesus that had been obscured by dogma. It imploded with the realization that separating “core” from “myth” was impossible, and conditioned a reactive “no-quest” in the first half of the twentieth century. RADITIONALLY CHRISTIANS have understood hat God's Kingdom is not of this world. It is not surprising, then, that history evinces some Christian difficulty in relating to thi's world. One aittitude takes ·a merely indirect interest in the world. Temporal activity is directed to the Church and its mission of saving souls. In this attitude the world has only an (...) instrumental value.1 Another attitude consists in a naive and innocent forgetfulness of temporal exigencies.2 A final one encompaisses 1 a disdain for temporal involvement.8 With its need for economic and political reform, the present century calls for a radical temporal engagement. For example, an indirect engagement by Catholics in the interest of the Church will not suffice. The Church shows an ability, even a resiliency, to exist in quite deficient temporal regimes. If the deficiencies of the present are to be remedied, it will be through the efforts of persons ·acknowledging more than an instrumental value to the temporal. Jacques Maritain, a Catholic layman, labored to establish the intellectual underpinnings for a radical Catholic engagement in the temporal. Maritian says, Humanism tends essentially to render man more truly human, and to manifest his original greatness by having him participate in all that which can enrich him in nature and in history...; it at once demands that man develop the virtualities contained within him, his creative forces and the life of reason, and work to make the forces of the physical world instruments of his freedom.4 That human nature contains such temporal capacities, Maritain later calls the "ontosophic truth." In his Peasant of the Garonne, Maritain speaks of the natural end of the world. By " world " Maritain especially understands " our human universe, the universe of man, of culture and history in their development here below." 5 The end of this world is three-fold.6 First is the mastery of nature by man and also the securement of freedom from servitude to other men. Second is the development of the spiritual activities of man, especially knowledge in the forms of wisdom and natural Finally, of " the manifestation of all the potentialities of human nature." How does a Christian come to make a radical commitment to the realization of these temporal capacities? It is not through any forsaking of his eternal destiny. Rather, for Maritain the temporal engagement follows in and through a deeper appreciation of that eternal destiny. The focus of this deeper appreciation is Christian sanctity. For Maritain only the saint is the true humanist. Why? Maritain concedes that sanctity is first and foremost a love of God. This case study describes the design, learning experiences, and student outcomes in one Instructional Design course with an explicit focus on globally competent teaching. We make the argument that forefronting global competence in an Instructional Design course, prior to social studies methods, is a necessary precursor to accelerate students’ progress on a pathway towards teaching for global competence. In support of this argument, we (a) describe the ways in which an Instructional Design course in one university forefronted global competence; (b) (...) explain the short- and long-term outcomes of this design; and (c) highlight four students to illustrate how the Instructional Design course helped to move students along a pathway towards global competence. The call has been made for global bioethics. In an age of pandemics, international drug trials, and genetic technology, health has gone global, and bioethics must follow suit. George Annas is one among a number of thinkers to recommend that bioethics expand beyond its traditional domain of patient–physician interactions to encompass a broader range of health-related matters. Medicine, Annas argues, must “develop a global language and a global strategy that can help to improve the health of all of the world's (...) citizens.” Individual countries cannot address global health issues, and culturally specific principles are inadequate for addressing global bioethics concerns. We will need a language and moral framework grounded in a foundation of universally shared, transcultural judgments about humankind that will also recognize moral pluralism. The claim has been made that such a foundation already exists in human rights, and that human rights should, therefore, be the new lingua franca of bioethics. (shrink)

Gaps in funded cancer medicines between New Zealand and Australia can have significant implications for patients and their families. Pharmac, the New Zealand pharmaceutical funding agency, has been criticised for not funding enough cancer medicines, and a 2022 review identified ethical concerns about its utilitarian focus on efficiency. However, as the costs of new cancer medicines rise along with public and political pressure to fund them, questions about value for money remain critical for health systems worldwide. In this paper, we (...) compare funding for cancer medicines in New Zealand and Australia, specifically medicines for non-small cell lung cancer. We argue that the ethical imperatives on funding agencies to get value for money and provide medicines for patients with cancer underscore the importance of transparent decision-making processes, including identifying and explaining intercountry differences in funded medicines. Background Obtaining informed consent in the clinical setting is an important yet challenging aspect of providing safe and collaborative care to patients. While the medical profession has defined best practices for obtaining informed consent, it is unclear whether these standards meet the expressed needs of patients, their families as well as healthcare providers. The authors sought to address this gap by comparing the responses of these three groups with a standardised informed consent paradigm. Methods Piloting a web-based ‘reverse’ simulation paradigm, (...) participants viewed a video showing a standardised doctor engaging in an informed consent discussion. The scenario depicted a simulated patient with psychotic symptoms who is prescribed an atypical antipsychotic medication. 107 participants accessed the simulation online and completed a web-based debriefing survey. Results Survey responses from patients, family members and healthcare providers indicated disparities in information retention, perception of the doctor's performance and priorities for required elements of the consent process. Conclusions To enhance existing informed consent best practices, steps should be taken to improve patient retention of critical information. Adverse events should be described in the short-term and long-term along with preventative measures, and alternative psychosocial and pharmacological treatment options should be reviewed. Information about treatment should include when the medication takes therapeutic effect and how to safely maintain the treatment. The reverse simulation design is a model that can discern gaps in clinical practice, which can be used to improve patient care. (shrink)

It is clear from George Annas's response to our arguments that he has misunderstood and misrepresented our positions on several key points. We suspect that this may be due in part to significant differences between our respective agendas and points of view, so we begin this exchange with an exploration of these differences.

: Pharmacogenetics offers the prospect of an era of safer and more effective drugs, as well as more individualized use of drug therapies. Before the benefits of pharmacogenetics can be realized, the ethical issues that arise in research and clinical application of pharmacogenetic technologies must be addressed. The ethical issues raised by pharmacogenetics can be addressed under six headings: regulatory oversight, confidentiality and privacy, informed consent, availability of drugs, access, and clinicians' changing responsibilities in the era of pharmacogenetic medicine. A rapidly growing proportion of health research uses ‘secondary data’: data used for purposes other than those for which it was originally collected. Do researchers using secondary data have an obligation to disclose individual research findings to participants? While the importance of this question has been duly recognised in the context of primary research, it remains largely unexamined in the context of research using secondary data. In this paper, we critically examine the arguments for a moral obligation to disclose individual (...) research findings in the context of primary research, to determine if they can be applied to secondary research. We conclude that they cannot. We then propose that the nature of the relationship between researchers and participants is what gives rise to particular moral obligations, including the obligation to disclose individual results. We argue that the relationship between researchers and participants in secondary research does not generate an obligation to disclose. However, we also argue that the biobanks or data archives which collect and provide access to secondary data may have such an obligation, depending on the nature of the relationship they establish with participants. The paper aims to show that the model of rationality dominating in our culture is based on a series of misunderstandings. According to the author this model consists of the following claims: (1) all our actions are caused by beliefs and desires; (2) rationality is a matter of following rules; (3) rationality is a separate cognitive faculty or capacity; (4) the weakness of the will can arise only in cases where there is something wrong with the psychological antecedent of the (...) action; (5) practical reason has to start with the primary desires that are beyond the scope of rationality; (6) rationality cannot be exercised if desires are inconsistent. The author discusses each of these claims, arguing that they are not tenable if we are to stay adequate to real cases of rational action. In particular the author develops an idea according to which rationality and freedom are coextensive, human beings are capable of commitment what creates desire-independent reasons for action, and desires can be inconsistent, for they constitute a kind of mental states different from beliefs: they represent not as the world is, but how we would like it to be. Background: A high prevalence of cognitive impairment and frailty complicates the feasibility of advance care planning in the long-term-care population. Research aim: To identify challenges in implementing the ‘Let Me Decide’ advance care planning programme in long-term-care. Research design: This feasibility study had two phases: (1) staff education on advance care planning and (2) structured advance care planning by staff with residents and families. Participants and research context: long-term-care residents in two nursing homes and one community hospital. Ethical considerations: The (...) local research ethics committee granted ethical approval. Findings: Following implementation, over 50% of all residents had completed some form of end-of-life care plan. Of the 70 residents who died in the post-implementation period, 14% had no care plan, 10% (with capacity) completed an advance care directive and lacking such capacity, 76% had an end-of-life care plan completed for them by the medical team, following discussions with the resident (if able) and family. The considerable logistical challenge of releasing staff for training triggered development of an e-learning programme to facilitate training. Discussion: The challenges encountered were largely concerned with preserving resident’s autonomy, avoiding harm and suboptimal or crisis decision-making, and ensuring residents were treated fairly through optimisation of finite resources. Conclusions: Although it may be too late for many long-term-care residents to complete their own advance care directive, the ‘ Let Me Decide’ programme includes a feasible and acceptable option for structured end-of-life care planning for residents with variable capacity to complete an advance care directive, involving discussion with the resident (to the extent they were able) and their family. While end-of-life care planning was time-consuming to deliver, nursing staff were willing to overcome this and take ownership of the programme, once the benefits in improved communication and enhanced peace of mind among all parties involved became apparent in practice. In my response I emphasize two main distinctions that may clear up some of the issues. One is the distinction between logical forms, in the sense of logical properties of an abstract character, and logical form, in the sense in which we speak of the logical form of a sentence, or of a proposition. Another is the distinction, emphasized (...) by Boole, between primary propositions , and secondary propositions —which I illustrate through the distinction between predicate negation and sentential negation.Em seu artigo John Corcoran examina em detalhe muitas questões sobre forma lógica e levanta alguns problemas relativos à minhas formulações. Na réplica enfatizo duas distinções principais, que podem esclarecer algumas questões. A primeira é a distinção entre formas lógicas, no sentido de propriedades lógicas de caráter abstrato, e forma lógica, no sentido em que falamos da forma lógica de uma sentença ou de uma proposição. In §1 I examine Boole’s “principle of wholistic reference” in relation to Frege’s postulation of truth-values as referents for sentences. I also consider in this connection Frege’s interpretation of quantification and his view that functions and concepts must be defined for all objects. I then present my own contrasting views on the reference of sentences. In §2 I discuss Boole’s introduction of the notion of universe of discourse and consider whether one of the issues implicit in John’s paper is (...) a confrontation between absolute interpretations of logic and relativistic interpretations of logic. O objetivo do presente trabalho é analisar a concepção de ciência de Popper recorrendo metáfora com o jogo de xadrez que ele utiliza em sua obra A lógica da pesquisa cientifica. Consideramos que por meio desta metáfora é possível percebermos que a sua concepção de ciência não se reduz ao critério lógico de falseabilidade como muitos de seus comentadores deixam entrever, nem tampouco, em definir regras metodológicas para nortear a prática da ciência. Mas, o próprio filósofo ao estabelecer tal analogia (...) deixa claro que a ciência tem em comum com o jogo de xadrez, a existência de regras. No nosso entender essas regras, assim como o critério de cientificidade, não são suficientes para definir a prática cientifica, pois esta envolve outros elementos como: valores, objetivos, comunidade cientifica e o contexto no qual as pesquisas são realizadas. Popper pressupõe esses elementos em sua metáfora, mas não os deixa explícitos. É somente em obras posteriores, como Conjecturas e refutações e Sociedade aberta e seus inimigos que eles são claramente definidos. Pretendemos assim, trazer a tona esses elementos envolvidos no jogo da ciência em Popper de modo a dar maior clareza a sua concepção. (shrink)

After introducing the field of somatics as a (...) human science, I first link it with Husserl’s earlier notion of somatology, then offer a transcendental critique of the root assumption of such a science – the psychophysical apperception – and trace the origin of the latter to the lived experience of death. Next I present Husserl’s notion of kinaesthetic consciousness as an alternative assumption for somatics, concluding with some contributions to a phenomenology of kinaesthetic systems as well as some suggestions for approaching somatic work in the spirit of embodied ethics and the kinaesthetics of respect.Key words: phenomenology, husserl, transcendental, somatics, psychophysical, lived body, kinaesthetic consciousness, kinaesthetic systems, embodied ethics. The fast pace of technological change necessitates new evaluative and deliberative tools. This article develops a general, functional approach to evaluating technological change, inspired by Nissenbaum’s theory of contextual integrity. Nissenbaum introduced the concept of contextual integrity to help analyze how technological changes can produce privacy problems. Reinterpreted, the concept of contextual integrity can aid our thinking about how technological changes affect the full range of human concerns and values—not only privacy. I propose a generalized concept of contextual integrity that (...) is applicable to a broader variety of circumstances, and I outline a new, general procedure for technological evaluation. Among the attractive features of the proposed approach to evaluating technological change are its context-sensitivity, adaptability, and principled presumptive conservatism, enabled by the mechanism the approach supplies for reevaluating existing practices, norms, and values.
